To arrange for enrollment in a Parent Information Program, fill in all the blanks on the following form.

You may be able to attend a scheduled class without enrolling prior to the class; however, a space may not be available. If space is available and you arrive a half hour prior to the start of the class with the necessary items you will be considered for admittance.
